Before Your First Appointment

Before your first appointment, you should complete the following:

1. Review the frequently asked questions which can answer many of the common questions that you may have.
2. Review the HIPAA Notice of Privacy Practices to understand our policy in maintaining your privacy.
3. Print and complete the 3 documents contained in the:
   New Client Forms
4. Bring the 3 New Client documents to your first appointment.
